Bird form is working now. Maybe it's just me, but I think it would be fun for something interesting to happen while you are filling out a form online. So I've remedied the situation.

It requires a browser that supports HTML5's canvas tag so IE9, Chrome, FF should work. I used Inkscape to draw the birds and then converted them from svg to javascript for canvas.

Yay birds!
